In this procedure, the surgeon creates a cut in the patient's back and then inserts a thin telescope into the kidney. The surgeon will then break the stone up and take it away. This process is used in cases of huge stones or in the event that the lithotripsy does not work.

Though rarely used, sometimes the surgeon also can make a cut in the side in order that he can reach the kidneys. This allows him to easily remove the stone. Such kidney stone treatment is scarcely needed.
